• Work Experience.
• Minimum of One year experience for Carers/support workers
• References (Name, Addresses, email and Telephone Number of 2 people who can provide you with references.  (This must be a charge nurse/sister/senior level staff).
• DBS Disclosure
• Bank details.
• P45 or equivalent.
• Proof of professional registration. NMC PIN NO. (nurses)
